Yesterday I did my usual 12km walk/run. I mostly power walk it as my shin hurts a lot when I run for 1km. I try to mix the walk, run and power walk just to get good cardio results. However this one was different and my route was longer than before. Anyhow good results and a great cardio day for me.

HOW IS IT DIAGNOSED?

As stated before, the shoulder is an extremely complex joint. There are many tissues that work together to help the shoulder function. The overwhelming majority of the time this is done perfectly, with no pain or significant weakness. But when things go wrong it is important to know what part(s) of the shoulder is/are affected.

The various physical tests that exist to test for shoulder injuries are notoriously unreliable on their own. A combination of the more reliable tests are needed in order to get a good quality diagnosis for a shoulder impingement, and which tissues are actually causing the pain.

Common tissues that cause pain are the:
- Supraspinatus tendon
- Long head of biceps tendon
- Subacromial bursa
- Acromion spur (Subacromial space narrowing)
- Rotator cuff tears
- AC joint sprains

When used together the most effective tests to diagnose shoulder impingement are the:
- Hawkins-kennedy
- Painful arc
- Infraspinatus strength
And if there is a rotator cuff tear suspected:
- Drop arm test
- Supraspinatus strength test
- Infraspinatus strength test

Usually this is enough to get a clear diagnosis of what is going on in the shoulder. Sometimes however, imaging is needed. Especially when a serious issue is suspected (fracture, labrum tear, etc). This month marked the halfway point of residency for me. 2.5 years down and another 2.5 to go. Celebrated by getting this new @medelita_gram white coat! I have received a lot of questions regarding training length for ortho. In general, most people will finish 4 years of undergraduate education, 4 years of medical school, 5-6 years of residency (depending on research), and 1 year of fellowship. Although this may seem like a lot, it all flies by much faster than you’d expect! .

Aparte de los músculos del útero, la musculatura más fuerte del cuerpo es la que cierra la mandíbula: por el contrario, los encargados de abrirla suelen estar muy débiles, y una apertura eficaz está supeditada al grado de tensión que presenten sus antagonistas, tensos y facilitados.
.
Al eliminar la tensión de la mandíbula relajando la musculatura sobresolicitada, provocamos un inmediato y agradable alivio sobre la tensión de cuello y hombros.
. 🎦1️⃣Abrir suavemente la mandíbula: Colocar cada dedo índice dentro del orificio del oído y abrir la boca: el espacio que se percibe dentro del oído se hace más grande debido a que el cóndilo mandibular se mueve hacia abajo y adelante (por eso oímos mejor con la boca abierta).
. 🎦2️⃣Milohioideo: colocar una mano en la posición de OMG! (😱) sobre la mejilla: la zona que asume el pulgar en esta posición es donde se encuentra este músculo. De entrada se percibe una estructura esponjosa (una glándula salival situada bajo la lengua, que puede que con el estímulo incremente su actividad) y con el movimiento del pulgar, suavemente liberamos su tensión.
. 🎦3️⃣Masetero: localizar el hueso de la mejilla, bajo la cavidad ocular, colocar los dedos debajo y deslizar suavemente hacia la curva mandibular.
.
🎦4️⃣Pterigoideo: colocando los dedos índices justo delante del oído, por delante del cóndilo mandibular, hay una pequeña depresión que se percibe mejor al abrir la mandíbula ligeramente: ahí se sitúa el pterigoideo. Lo liberamos con suave masaje circular, manteniendo esa ligera apertura. A continuación lo estiramos colocando las manos en el cráneo y dejando que la cabeza vaya hacia atrás con la mandíbula semiabierta: manteniendo atrás la cabeza, movemos suavemente la mandíbula adelante y atrás y finalmente recuperamos la posición de la cabeza con la ayuda de las manos.
. 🎦5️⃣Temporal : localizamos el punto donde se inserta, justo bajo el arco cigomático: es normal percibir mucha tensión en este punto.

🎦6️⃣Liberación con los dedos del SCM y flexión lateral para estirarlo partiendo de extensión desde la base del cráneo.

Al eliminar la tensión de la mandíbula relajando la musculatura sobresolicitada, provocamos un inmediato y agradable alivio sobre la tensión de cuello y hombros.
. 🎦1️⃣Abrir suavemente la mandíbula: Colocar cada dedo índice dentro del orificio del oído y abrir la boca: el espacio que se percibe  dentro del oído se hace más grande debido a que el cóndilo mandibular se mueve hacia abajo y adelante (por eso oímos mejor con la boca abierta).
. 🎦2️⃣Milohioideo: colocar una mano en la posición de OMG! (😱) sobre la mejilla: la zona que asume el pulgar en esta posición es donde se encuentra este músculo. De entrada se percibe una estructura esponjosa (una glándula salival situada bajo la lengua, que puede que con el estímulo incremente su actividad) y con el movimiento del pulgar, suavemente liberamos su tensión.
. 🎦3️⃣Masetero: localizar el hueso de la mejilla, bajo la cavidad ocular, colocar los dedos debajo y deslizar suavemente hacia la curva mandibular.
.
🎦4️⃣Pterigoideo: colocando los dedos índices justo delante del oído, por delante del cóndilo mandibular, hay una pequeña depresión que se percibe mejor al abrir la mandíbula ligeramente: ahí se sitúa el pterigoideo. Lo liberamos con suave masaje circular, manteniendo esa ligera apertura. A continuación lo estiramos colocando las manos en  el cráneo y dejando que la cabeza vaya hacia atrás con la mandíbula semiabierta: manteniendo atrás la cabeza, movemos suavemente la mandíbula adelante y atrás y finalmente recuperamos la posición de la cabeza con la ayuda de las manos.
. 🎦5️⃣Temporal : localizamos el punto donde se inserta, justo bajo el arco cigomático: es normal percibir mucha tensión en este punto.
.
🎦6️⃣Liberación con los dedos del SCM y flexión lateral para estirarlo partiendo de extensión desde la base del cráneo.
